Ways to Participate

Choose the option that works best for you. Both make a meaningful difference.

Registry

Share your health information through online surveys from the comfort of your home.

What's Involved:
  • Complete surveys about your health and daily life
  • Takes about 30-45 minutes to start, then only 5-10 minutes each month
  • All done online at your convenience
  • No in-person visits required
Who Can Join:
  • Age 30 or older
  • Diagnosed with heart failure or at risk
  • Able to complete online surveys

Deep Phenotyping

Contribute more detailed information through in-person visits and additional testing. Note: Deep Phenotyping is only available to participants who meet specific eligibility criteria.

What's Involved:
  • Visit a medical center near you
  • Complete detailed health assessments
  • Provide blood samples and other tests
  • Optional: Contribute tissue samples
  • Follow-up visits over time
Who Can Join:
  • Age 30 or older
  • Specific heart failure diagnosis (HFpEF) required
  • Able to visit a participating medical center
  • Meet additional health criteria determined by study team

Deep Phenotyping has more restrictive eligibility requirements. Most participants will join through the Registry option.

How to Join

Getting started is simple. Here's what to expect:

Fill out a brief form online or call us. We'll explain the options and answer your questions.

We'll send you detailed information about what participation involves and what to expect.

Review all the information and ask any questions. You'll sign consent forms when you're ready.

Begin with your first survey or visit. Our team will guide you through every step.

Common Questions

How much time does it take?
Registry participants spend about 30-45 minutes on the first set of surveys, then only 5-10 minutes each month after that. Deep Phenotyping involves a two-day initial visit, with shorter follow-up visits scheduled yearly.
Will I be compensated?
Registry participants are entered into a quarterly raffle to receive gift cards for completing surveys. Deep Phenotyping participants receive compensation for each visit. Additional compensation may be available for travel expenses.
Is my information private?
We take every precaution to secure your health information. We follow strict privacy rules and only share de-identified data with approved researchers.
Can I stop participating?
Yes, you can withdraw at any time for any reason. Your decision won't affect your regular medical care in any way.
Will I get my test results?
Deep Phenotyping participants receive clinically relevant test results. Registry participants don't receive individual results, but everyone can get updates about what we're learning from the community here on our website.
