Share your health information through secure online surveys — entirely from home, on your own schedule.

In-Depth Research

Deep Phenotyping

Contribute more detailed information through in-person visits and comprehensive assessments at a medical center near you.

  • Visit a participating medical center
  • Complete detailed health assessments
  • Provide blood samples and other tests
  • Optional: contribute tissue samples
  • Follow-up visits over time
  • Age 30 or older
  • HFpEF diagnosis required
  • Able to visit a participating medical center
  • Meet additional criteria set by study team

How much time does it take?
Registry participants spend about 30–45 minutes on the first set of surveys, then only 5–10 minutes each month after that. Deep Phenotyping involves a two-day initial visit, with shorter follow-up visits scheduled yearly.
Will I be compensated?
Registry participants are entered into a quarterly raffle for gift cards for completing surveys. Deep Phenotyping participants receive compensation for each visit. Additional compensation may be available for travel expenses.
Is my information private?
We take every precaution to secure your health information. We follow strict privacy rules and only share de-identified data with approved researchers.
Can I stop participating?
Yes — you can withdraw at any time for any reason, no questions asked. Your decision will never affect your regular medical care.
Will I get my test results?
Deep Phenotyping participants receive clinically relevant test results. Registry participants don't receive individual results, but everyone can get updates on what the community is learning on our website.
